Notice of Practice Closure
Dear Patient,
After careful consideration, Life Psychiatry will permanently close on September 25, 2026. Our official 30-day notice period begins August 25, 2026.
Serving you has been an incredible privilege, and we are deeply grateful for the trust you placed in us. We understand that transitioning care can be challenging, and we are committed to making this process as smooth as possible.
Key dates
– August 25, 2026: 30-day closure notice period begins
– September 18, 2026: Final day for patient appointments
– September 25, 2026: Practice permanently closes
Continuity of care & final visits
To support your care transition, we are offering 10-minute Final Transition & Refill Visits through September 18, 2026. If you have an upcoming appointment or need a temporary medication bridge, please call 251-999-5433 to confirm or schedule on a first-come, first-served basis.
Please note that no medication changes, dose adjustments, or new evaluations will be made during these final visits. Because our practice is closing, these brief appointments are designated strictly to review your active medications, provide your Psychiatric Care Summary for your new doctor, and issue a short-term bridge refill to prevent gaps in care.
Note: All bridge refills are limited strictly to existing prescriptions on file, are reviewed individually by your treating clinician, and remain subject to clinical judgment, legal requirements, and clinical appropriateness. Controlled substances remain subject to all applicable prescribing laws. A bridge refill is not guaranteed.
Establishing care with a new provider
We recommend establishing care with a new psychiatry provider or primary care provider (PCP) as soon as possible.
Some of our providers will be available at new practice locations:
– Courtney Riggs, PA-C: Available at Wellness Collective starting September 21, 2026. Call 251-480-0070 to schedule.
– Kevin Dudeck, LPC: Available for existing therapy patients at Life Psychiatry until September 18, 2026. Call 251-999-5433 to schedule. Available after closure via Headway.co and Psychology Today.

Your medical records
Your medical records will remain confidential and securely maintained. To ensure a seamless transition, please review your options for obtaining your medical records:
Psychiatric Care Summary (free)
– Best for: Establishing care quickly with a new doctor or PCP
– Includes: Provider, diagnoses, and medications
– Turnaround: Typically a few business days
– How to get it: Provided at your final visit or call 251-999-5433
Full medical record
– Best for: Legal matters or a complete archive
– Includes: Complete history of treatment notes
– Turnaround: Up to 30 days, depending on volume
– How to get it: Call 251-999-5433 to be sent a Medical Records Authorization Form
Under Alabama law, records are retained for at least 7 years from your last visit (longer for minors). If we do not receive a records request or transfer instructions from you, your records will remain securely maintained by Life Psychiatry or its designated records custodian in accordance with Alabama law, and may be securely destroyed after the applicable retention period. After closure, medical record requests will be managed by our custodian at LiveFreeMobile.com/records.
