New patient onboarding

All new patients are required to go through both steps of the New Patient process, outlined below.

Part I: New Patient Intake Consultation

During your first visit, you’ll have a one-on-one intake with a staff member to discuss your treatment goals and gather your full history, experiences and challenges. This information is a critical input into the second half of the New Patient process, which is the Psychiatric Evaluation. Medications cannot be prescribed during this appointment. You’ll meet with a staff member (not Dr. Diaz).

55-minute one-time session:
– $180 self-pay
– Eligible to be billed to BCBS

Part 2: New Patient Psychiatric Evaluation

During your second visit, board-certified psychiatrist Dr. Diaz will perform a thorough psychiatric evaluation to arrive at an initial diagnostic impression and work with you to create a personalized treatment plan for your needs. Dr. Diaz employs medication only when necessary, taking the time to explain its physiological effects and collaborating with you to ensure it aligns with your psychiatric goals.

55-minute one-time session:
$340 self-pay

Services for established patients

Medication Management

As you move forward with your psychiatric treatment plan, medication management follow-ups allow us to evaluate or adjust your medications, while spending the bulk of the session focused on individualized psychotherapy. You’ll meet with Dr. Diaz, MD every 10-14 days until you are stable; then every 1-3 months to ensure safe, effective care.

25-minute follow up sessions:
– $190 self-pay
– Eligible to be billed to BCBS

Therapy with a Licensed Therapist

Our therapy sessions create a safe, caring space where you can explore your thoughts and feelings, develop new coping strategies, and grow in a way that feels right for you. You’ll meet with a licensed provider like Kevin Dudeck, LPC who specializes in anxiety, depression, anger management, life transitions, addiction, family conflict, self-esteem, and more.

55-minute sessions:
– $160 self-pay
– Eligible to be billed to BCBS

Medication Refill Processing Between Appointments

It is your responsibility to manage your available medications. If your prescription will run out before your next appointment, you can request a medication refill online and call the office. We’ll charge your payment method on file and process your request within 1-3 business days. The request must be for the same medication and dosage. Your previous appointment must have been less than 3 months ago, and a future appointment scheduled.

Online refill fee:
$55 self-pay
– Not eligible to be billed to insurance
because it is outside of your office appointment

Virtual Telehealth Appointments

Life Psychiatry offers telehealth appointments for established patients, providing convenient care via a secure, HIPAA-compliant platform. You must be located in Alabama for the session, and availability varies by provider.

There is no additional fee for telehealth capabilities but eligibility is discussed on a case-by-case basis

Spravato® treatment for depression

Spravato (esketamine) is a nasal spray treatment approved for patients with treatment-resistant depression, offering rapid relief when other antidepressants have not been effective. Administered under the supervision of a healthcare provider, Spravato helps improve mood and symptoms by targeting brain receptors to restore balance in mood-regulating neurotransmitters. Spravato is most effective when paired with regular therapy.

Additional information:
– Spravato treatment services are for established psychiatric patients of Life Psychiatry only
– Dr. Diaz will oversee treatment but may not be actively involved in weekly treatment sessions
– Dosage, frequency and treatment progress is discussed in Medication Management appointments

Spravato Start Up Sessions (First 4 weeks)

When starting Spravato, patients will be closely monitored for about two hours to ensure their safety and comfort as the medication takes effect. During the induction phase, treatment is scheduled twice a week for the first four weeks. Dosage and potential side effects will be discussed with your provider.

2-hour treatment sessions:
$320 self-pay administering fee per session
– Medical administering can be billed to BCBS, but the Spravato medication cost depends on your pharmacy benefits

Spravato Maintenance Sessions

During the maintenance phase, Spravato treatment is typically scheduled once a week depending on the patient’s response to the medication. For patients maintaining a stable response, the frequency can be reduced to every two weeks or once a month.

2-hour treatment sessions:
$320 self-pay administering fee per session
– Medical administering can be billed to BCBS, but the Spravato medication cost depends on your pharmacy benefits

Specialized services (for new or established patients)

Fees for Administrative Forms, Letters & Reports (Self-Pay only)

Dr. Diaz charges an administrative fee for forms, letters, or psychiatric assessment reports requested outside of appointments, covering the time, effort, and licensing involved. This fee is not billable to insurance and applies to requests like school letters, employment paperwork, and disability forms. Each request is evaluated on a case-by-case basis, with an estimated time and cost provided. Payment is required before proceeding.

Patients who have been treated for 4 months or more and have had at least 4 visits:
– $65 per 15-minute increment
– Not eligible to be billed to insurance

New patients or those with fewer than 4 visits and/or less than 4 months of treatment:
– $450 for the first hour (1 hr minimum for non-established patients), with $100 for each additional 15-minute increment
– Not eligible to be billed to insurance

Life Coaching Sessions with Dr. Diaz (Self-Pay only)

Life can be overwhelming, especially when navigating complex family dynamics or facing crucial life decisions. Dr. Diaz provides a professional and supportive space for both current and new patients, including parents and family members in urgent need of guidance for a troubled loved one. Whether you’re confronting relationship challenges, family issues, or are uncertain about your next steps, Dr. Diaz offers the clarity, support, and practical solutions necessary to move forward. Available on a self-pay basis, this non-medical service cannot be billed to insurance.

55-minute sessions for new or established patients:
– $300 self-pay
– Not eligible to be billed to insurance or superbills

Fast-Track Psychiatric Evaluation with Psychiatric Fitness Assessment Report (Self-Pay only)

Dr. Diaz offers a “fast-track” psychiatric evaluation that includes a detailed assessment of your mental health, medical history, and current symptoms. Within 5-7 business days, you’ll receive a report evaluating your fitness for work, school, service, or the court system. This service is available on a self-pay basis due to the time, effort, and medical expertise required to complete the report. You will meet with Dr. Diaz and you do not need to be a patient at Life Psychiatry. The report will be completed within 7-10 business days, and sent to you or an authorized party.

90-minute one-off session for new patients or those with fewer than 4 visits and/or less than 4 months of treatment:
– $850 self-pay
– Not eligible to be billed to BCBS

For patients who have been treated for 4 months or more and have had at least 4 visits:
– $300 self-pay for 90-min written report
– Not eligible to be billed to insurance
