New Patient Intake Consultation
During your first visit, you’ll have an initial one-on-one intake consultation where we will take the time to get to know you in depth—exploring your history, background, past experiences, and challenges.

– Barbara Turner, Nurse Practitioner

This appointment is required to become a Life Psychiatry patient.
Psychiatric Evaluation
During your second visit, Dr. Diaz, M.D., board-certified psychiatrist, will perform a thorough psychiatric evaluation to arrive at an accurate diagnosis. Based on this diagnosis, a personalized treatment plan will be developed to address your specific needs.
You’ll meet with:
– Dr. Diaz, M.D., board-certified psychiatrist
60 mins
$340 self-pay or BCBS visit
(Eligible to be billed to BCBS)
This appointment is required to become a Life Psychiatry patient.
Medication Follow Ups
As you move forward with your treatment plan, medication management follow-ups allow us to evaluate the effectiveness of your medications and track your progress over time.
You’ll meet with:
– Dr. Diaz, M.D., board-certified psychiatrist; or
– Barbara Turner, Nurse Practitioner
45-60 mins
$180-280 self-pay or BCBS visit (e.g. co-pay)
(Eligible to be billed to BCBS)
It is our policy to conduct follow-up appointments every 3 months to ensure the delivery of safe and appropriate care.
Counseling
Our counseling services provide a safe, supportive space to explore personal challenges, improve mental health, and develop coping strategies. We offer personalized guidance to help you navigate life’s difficulties and foster emotional well-being.
You’ll meet with:

$120-150 self-pay or BCBS visit (e.g. co-pay)

Counseling is optional but recommended. We also offer faith-based Christian counseling.
