Insurance policy
Life Psychiatry is closer to fully accepting Blue Cross Blue Shield insurance. The first new patient appointment is self-pay ($160), but subsequent appointments with Dr. Diaz are eligible to be billed to BCBS. Please let the front desk know if you’d like to use your BCBS insurance.
We are still waiting for acceptance into Medicaid’s network.
We understand that paying out-of-pocket is not possible for everyone; and we are very sorry for the inconvenience.

Rates
New Patient Intake Consultation
During your first visit, you’ll have an initial one-on-one intake consultation where we will take the time to get to know you thoroughly — your history, background, past experiences, and challenges. This appointment is required to become a Life Psychiatry patient.

Psychiatric Evaluation
In your second visit, Dr. Diaz will conduct an in-depth psychiatric evaluation to effectively get to a diagnosis. With a clinical diagnosis, a customized treatment plan will be created that is tailored to your unique needs. This appointment is required to become a Life Psychiatry patient.

Medication Management / Follow ups
As you progress through your treatment plan, it’s through follow ups, medication management (if needed), and therapy that we can measure your progress and improvement over time.
