New patient onboarding
All new patients must complete the two-part patient onboarding process, outlined below.
Part I: New Patient Intake Consultation
During your first visit, you’ll have a one-on-one intake with a staff member to discuss your treatment goals and gather your full history, experiences and challenges. This information is a critical input into the second half of the New Patient process, which is the Psychiatric Evaluation. Medications cannot be prescribed during this appointment. You’ll meet with a staff member (not Dr. Diaz).
55-minute one-time session:
– $180 self-pay with staff member
– Eligible to be billed to BCBS
Part 2: New Patient Psychiatric Evaluation
During your second visit, we will perform a thorough psychiatric evaluation to arrive at an initial diagnostic impression and work with you to create a personalized treatment plan for your needs. We will employ medication only when necessary, taking the time to explain its physiological effects and collaborating with you to ensure it aligns with your psychiatric goals.
55-minute one-time session:
– $340 self-pay with Dr. Diaz
– $300 self-pay with PA, NP
– Eligible to be billed to BCBS
Established patient services
As an established patient, you’ll have access to the following services and benefits.
Medication Management
As you move forward with your psychiatric treatment plan, medication management follow-ups allow us to evaluate or adjust your medications, while spending the bulk of the session focused on individualized psychotherapy. You’ll meet with your provider every 10-14 days until you are stable; then every 1-3 months to ensure safe, effective care.
25-minute follow up sessions:
– $190 self-pay with Dr. Diaz
– $168 self-pay with PA, NP
– Eligible to be billed to BCBS
Therapy with a Licensed Provider
Our psychotherapy sessions create a safe, caring space where you can explore your thoughts, develop new coping strategies, and grow in a way that feels right for you. You’ll meet with a licensed provider like Kevin Dudeck, LPC who specializes in anxiety, depression, anger management, and more; or Courtney Riggs, PA who specializes in self-esteem, addiction, life transitions, and more.
55-minute sessions:
– $160 self-pay with LPC, PA, NP
– Eligible to be billed to BCBS
Spravato® treatment for depression
Spravato (esketamine) is a nasal spray treatment approved for patients with treatment-resistant depression, offering rapid relief when other antidepressants have not been effective. Spravato helps improve mood and symptoms by targeting brain receptors to restore balance in mood-regulating neurotransmitters.
Sessions are 2 hours long. Medical administering can be billed to BCBS, but Spravato medication costs depend on your pharmacy benefits. Self-pay options not available due to the difficulty of accessing Spravato medication without pharmacy insurance benefits.
Treatment schedule:
– “Induction Phase” first 1-4 weeks: 2 treatments per week
– “Maintenance Phase” week 5+: 1 treatment per week depending on the patient’s response to the medication
Virtual Telehealth Appointments
Life Psychiatry offers telehealth appointments for established patients, providing convenient care via a secure, HIPAA-compliant platform. You must be located in Alabama for the session, and availability varies by provider.
Eligibility is discussed on a case-by-case basis.
There is no additional fee for telehealth capabilities.
Specialized services
“Fast-Track” Psychiatric Evaluation & Report (Self-Pay only)
Life Psychiatry offers a “fast-track” psychiatric evaluation that includes a detailed assessment of your mental health, medical history, and current symptoms. This service is available on a self-pay basis due to the time, effort, and medical expertise required to complete the report. The report will be completed within 7-10 business days, and sent to you or an authorized party.
90-minute one-off session for new patients or those with fewer than 4 visits and/or less than 4 months of treatment:
– $1,250 self-pay with Dr. Diaz
– Not eligible to be billed to BCBS
For patients who have been treated for 4 months or more and have had at least 4 visits:
– $400 self-pay for written report
– Not eligible to be billed to insurance
Life Coaching with Dr. Diaz (Self-Pay only)
Life can be overwhelming, especially when navigating complex family dynamics or facing crucial life decisions. Dr. Diaz provides a professional and supportive space for both current and new patients, including parents and family members in need of guidance for a troubled loved one. Whether you’re confronting relationship challenges, family issues, or are uncertain about your next steps, Dr. Diaz offers the clarity, support, and practical solutions necessary to move forward. Available on a self-pay basis, this non-medical service does not include medication management and the session cannot be billed to insurance.
55-minute sessions for new or established patients:
– $340 self-pay
– Not eligible to be billed to insurance or superbills
Clinic fees
Administrative Fees
Due to the time-intensive nature of medical paperwork, all forms, letters, and psychiatric assessment reports incur a time-based administrative fee. Each request is evaluated on a case-by-case basis, with an estimated time and cost provided. Payment is required before proceeding.
Patients treated for 4 months and attended at least 4 visits:
– $65 per 15-minute increment
– Not eligible to be billed to insurance
New patients with less than 4 visits or less than 4 months of treatment:
– $450 for the first hour (1 hr minimum), and $100 for each additional 15-minute increment
– Not eligible to be billed to insurance
Medication Refills Between Appointments
If your prescription runs out before your next appointment, you can request a medication refill online. We’ll charge your payment method on file and process the request within 1-3 business days. The request must be for the same medication and dosage.
Online refill fee:
– $55 self-pay
– Not eligible to be billed to insurance because it is outside of an office appointment
Medical Records Processing
Fee for medical records processing to cover the administrative time and resources required to securely handle, review, and prepare medical documents. This includes retrieving records, verifying authorization, ensuring compliance with privacy regulations, and delivering records in a timely manner.
Processing & transmission fee:
– $1 per page up to 25 pages; then $0.50 per page thereafter
– Not eligible to be billed to insurance
