Accepting new patients

Life Psychiatry is accepting new patients and we’re glad that you’re considering becoming a patient with us. Start by completing the New Patient form. Once completed, call the office and provide your full name and date of birth to schedule your first appointment with a staff member. Step 1

Fill out the “New Patient” form

Approximate time to complete: 10-15 minutes

Complete the “New Patient” form in order to schedule your first appointment at Life Psychiatry. All new patients must fill out this required form and place a payment method on file. Begin by entering your name and email or phone number. You will then receive a code via email or text to access the form.

Gather this information before starting the form:
– Your full medical and medication history
– Your government ID
– Your insurance card
– Your credit card/payment information for appointments and cancellation fees (required to schedule an appointment)
– Preferred pharmacy

Step 2

Schedule your New Patient appointments

Once your new patient forms are complete, call 251-999-5433 to schedule your two-part New Patient appointments. Both sessions are required to get an in-depth history about you and to go through various medical DSM-5 criteria for diagnosis. The first and second appointments cannot be done on the same day.

We suggest that you bring a list of your current medications and any medical information or records that you think will be helpful. Also bring your doctor’s referral if your insurance company requires a referral to see a psychiatrist or specialist.

If you need to cancel or reschedule any appointments, please give us 48 hours notice otherwise you will be charged a fee of $100.

What to expect in your appointments

Life Psychiatry implements a three-part mental health treatment process:

1st Appointment:
New Patient Initial Consultation

During your first visit, you’ll have a one-on-one intake with a staff member to discuss your treatment goals and gather your full history, experiences and challenges. This information is a critical input into the second half of the New Patient process, which is the Psychiatric Evaluation. Medications cannot be prescribed during this appointment.

Duration: 55 minutes

Who will I meet with? This session will be with a licensed staff member. The first appointment cannot be made with Dr. Diaz.

2nd Appointment:
Psychiatric Evaluation

During your second visit, we will perform a thorough psychiatric evaluation to arrive at an initial diagnostic impression and work with you to create a personalized treatment plan for your needs. We will employ medication only when necessary, taking the time to explain its physiological effects and collaborating with you to ensure it aligns with your psychiatric goals.

Duration: 55 minutes

Who will I meet with? This session will be with Dr. Diaz, MD or Courtney Riggs, PA-C.

Established Patients:
Medication Follow Ups & Therapy

All patients are different and will see progress over different timeframes but it’s through regular medication management (if needed) and therapy sessions that we can measure your progress and improvement over time.

Duration:
– 25 minute Medication Management
– 55 minute Therapy sessions
