How to Become a Patient
We’re glad that you’re considering becoming a patient at Life Psychiatry. Start by completing the New Patient form. Once completed, call the office and provide your full name and date of birth to schedule your first appointment with a staff member. Please refer to the Services & Rates or Insurance pages to learn more about our services and coverages.
Step 1
Fill out the “New Patient” form
Approximate time to complete: 10-15 minutes
Complete the “New Patient” form in order to schedule your first appointment at Life Psychiatry. All new patients must fill out this required form and place a payment method on file. Begin by entering your name and email or phone number. You will then receive a code via email or text to access the form.
Gather this information before starting the form:
– Your full medical and medication history
– Your government ID
– Your insurance card
– Your credit/payment information for co-pays and cancellation fees (must be placed on file to schedule an appointment)
– Preferred pharmacy
Step 2
Schedule your New Patient appointments
Call 251-999-5433 to schedule your appointments.
The 1st appointment is the “New Patient Intake Consultation” with a staff counselor:
– What to expect: During your first visit, you’ll have a 1-hour one-on-one intake consultation with a Life Psychiatry staff member where we will take the time to get to know you thoroughly — your history, background, past experiences, and challenges. It may be difficult to share so much about yourself with us, but we assure you that the more history you can provide, the better we’ll be able to work with you to move forward.
– How much does it cost and will it be covered by my insurance? Please see services & rates.
– What to bring: Bring a list of your current medications and any medical information or records that you think will be helpful. Bring your doctor’s referral document if your insurance company requires a referral to see a psychiatrist or specialist.
– Who will I meet with and how long will it take? This session will be with a staff counselor and is 55 minutes long. The first appointment cannot be made with Dr. Diaz.
The 2nd appointment is the “Psychiatric Evaluation” with Dr. Diaz:
– What to expect: During your second visit, Dr. Diaz will perform a thorough psychiatric evaluation to arrive at an accurate diagnosis. A personalized treatment plan will be developed to address your specific needs. Your personalized mental health plan may, or may not, include medications. It also may or may not include psychotherapy to help process trauma or give you the tools to deal with stress, anxiety, and depression with or without medication.
– How much does it cost and will it be covered by my insurance? Please see services & rates.
– What to bring: Any additional information that you believe will be helpful for your diagnosis, including your list of medications or previous medical records.
– Who will I meet with and how long will it take? This session is with Dr. Diaz and is about 50 minutes long.
Both appointments are required to become a Life Psychiatry patient and they cannot be done on the same day. View Services & Rates to view rates. If you need to cancel or reschedule any appointments, please give us 48 hours notice otherwise you will be charged a fee of $100.

Life Psychiatry implements a three-part mental health treatment process for established patients:
Evaluation:
New Patient Initial Consultation
As a new patient, we will take the time to get to know you thoroughly. We will have an in-depth conversation about your history, background, past experiences, and challenges. Through a series of questions and assessments, we aim to understand how your body and brain function. We work with both children and adults and provide high-quality care to all patients.
Diagnosis & Treatment:
Psychiatric Evaluation
With a clinical diagnosis, we can create a customized treatment plan that is tailored to your unique needs and experiences. Dr. Diaz employs medication only when necessary, taking the time to explain its physiological effects and collaborating with you to ensure it aligns with your psychiatric goals. Paired with cognitive behavioral therapy, counseling, and real-talk to get to the root of issues, we will see healing and progress toward a better future.
Maintenance:
Medication Follow Ups & Counseling
All patients are different and will see progress over different timeframes but it’s through regular medication management (if needed) and counseling that we can measure your progress and improvement over time.
