Services we provide: At Life Psychiatry, we assess, diagnose, and treat a wide range of mental health conditions such as ADHD, autism, anxiety, depression, and mood disorders. View all conditions we treat.

Services we do not provide: Because our #1 focus is on treatment and recovery, we unfortunately do not complete forms or letters for disability, legal, employment, or insurance matters. We also do not offer formal testing services such as IQ tests or neuropsychological assessments. We recommend seeking a specialized provider for such documentation or tests. Thank you for your understanding.

New patient onboarding

All new patients must complete the two-part patient onboarding process, outlined below.

Part I: New Patient Intake Consultation

In your first visit, you’ll have a one-on-one intake with a staff member to discuss your treatment goals and gather your full history, experiences and challenges. This information is a key input into the second half of the New Patient process, which is the Psychiatric Evaluation. Medications cannot be prescribed during this appointment. You’ll meet with a staff member (not Dr. Diaz).

55-minute one-time session:
– $180 self-pay with staff member (LPC, PA, NP)
– Eligible to be billed to BCBS

Part 2: New Patient Psychiatric Evaluation

During your second visit, we will perform a thorough psychiatric evaluation to arrive at an initial diagnostic impression and work with you to create a personalized treatment plan for your needs. We will employ medication only when necessary, taking the time to explain its physiological effects and collaborating with you to ensure it aligns with your psychiatric goals.

55-minute one-time session:
– $340 self-pay with Dr. Diaz
– $300 self-pay with PA, NP
– Eligible to be billed to BCBS

Established patient services

As an established patient, you’ll have access to the following services and benefits.

Medication Management

As you move forward with your psychiatric treatment plan, medication management follow-ups allow us to evaluate or adjust your medications, while spending the bulk of the session focused on individualized psychotherapy. You’ll meet with your provider every 10-14 days until you are stable; then every 1-3 months to ensure safe, effective care.

20-25 minute full follow up sessions:
– $190 self-pay with Dr. Diaz
– $168 self-pay with PA, NP
– Eligible to be billed to BCBS

10-12 minute medication management only sessions:
– $150 self-pay with Dr. Diaz
– $128 self-pay with PA, NP
– Eligible to be billed to BCBS

Therapy / Counseling

Our psychotherapy sessions create a safe, caring space where you can explore your thoughts, develop new coping strategies, and grow in a way that feels right for you. You’ll meet with a licensed provider like Kevin Dudeck, LPC who specializes in anxiety, depression, anger management, and more; or Faith Farmer, counseling intern, who offers support in anxiety, depression, life transitions, addiction recovery, identity, self-esteem.

55-minute sessions:
– $160 self-pay with LPC, PA, NP
(eligible to be billed to BCBS)
– $80 self-pay with Counseling Intern (self-pay only; not eligible to be billed to BCBS)

Spravato® treatment for depression

Spravato (esketamine) is a nasal spray treatment approved for patients with treatment-resistant depression, offering rapid relief when other antidepressants have not been effective. Spravato helps improve mood and symptoms by targeting brain receptors to restore balance in mood-regulating neurotransmitters.

Sessions are 2 hours long. Medical administering can be billed to BCBS, but Spravato medication costs depend on your pharmacy benefits. Self-pay options not available due to the difficulty of accessing Spravato medication without pharmacy insurance benefits.

Treatment schedule:
– “Induction Phase” first 1-4 weeks: 2 treatments per week
– “Maintenance Phase” week 5+: 1 treatment per week depending on the patient’s response to the medication

Virtual Telehealth Appointments

Life Psychiatry offers telehealth appointments for established patients, providing convenient care via a secure, HIPAA-compliant platform. You must be located in Alabama for the session, and availability varies by provider.

Eligibility is discussed on a case-by-case basis.

There is no additional fee for telehealth capabilities.

Specialized services

Medical Consultation or Life Coaching with Dr. Diaz (Self-Pay only)

Life can feel overwhelming when dealing with personal uncertainty, family tension, or supporting a loved one through crisis. Dr. Diaz offers supportive sessions for individuals or family members who need guidance. These self-pay consultations are available to both new and current patients. No intake appointment is required. This is not a medical visit, and medications will not be prescribed. The session cannot be billed to insurance.

55-minute one-off session:
– $340 self-pay with Dr. Diaz
– $100 refundable deposit required
– Not eligible to be billed to insurance

Clinic fees

Administrative Fees

Life Psychiatry focuses on treatment and recovery, not paperwork for disability, legal, employment, or insurance matters. We do not complete forms or letters for those purposes and recommend seeking a specialized provider for such documentation. We may offer brief treatment summaries or school-related letters if clinically appropriate.

All patient-requested letters or paperwork incur a time-based administrative fee of $100 per 30 minutes. We do not process paperwork for patients with less than 3 appointments completed.

Medical Records Processing

Our providers do not release patient evaluations, progress notes or therapy notes. A summary of care will be provided to patients and/or third parties when medical records are requested.

Records processing fee:
– $1 per page up to 25 pages; $0.50 per page thereafter
– Not eligible to be billed to insurance
– Pre-payment required
– Requests are only processed on Fridays and have a turnaround time of up to 21 days

Medication Refills Between Appointments

If your prescription runs out before your next appointment, you can request a medication refill online. We’ll charge your payment method on file and process the request within 1-3 business days. The request must be for the same medication and dosage.

Online refill fee:
$20 self-pay
– Not eligible to be billed to insurance because it is outside of an office appointment
